Abstract
This paper aims to synthesize the history of 6 decision biases: confirmation bias, hindsight bias, anchoring bias, recency bias, primacy effect, and groupthink. Each chapter addresses the history of the bias, moving into the causes, and presenting the bias in different historical events including the Challenger Disaster (1986), Mount Everest (1996), and the Titan Submersible Implosion (2023). The final chapter summarizes prevention techniques for each bias, as well as universal solutions to implement in organizations. These techniques are essential to reducing the role of biases and making stronger, more educated decisions.
